Safe Route to School Survey

The Village of Kincaid is applying for a grant to remove and replace sidewalks on our children's routes to school and has teamed up with the South Fork School District to gather data for that grant.
Your input is extremely important and this survey will only take about 3 to 5 minutes to complete. We ask that each family complete only one survey per school that your children attend. If you have children who attend both schools, please fill the form out twice.
Your responses will be kept confidential and neither your name nor your child's name will be associated with any results.
